Jane Kealey is the Telehealth Manager at Northeast Health Wangaratta, in regional north east Victoria. Jane has worked in telehealth for 7 years, designing clinical workflows enhanced by technology to address clinical service needs in regional settings. With a background in metro and rural nursing, along with research in cardiovascular and rural health, Jane’s motivation is to contribute to the reduction of the rural-metro health divide through greater access to health and wellness care.

This forum will reflect on a sub-regional public health services’ experience of implementing a remote patient monitoring project in an environment where change requirements ultimately could not be met. Significant effort was made by the range of stakeholders recognising benefits offered to patients to improve their self-management of chronic disease. The experience and learnings (to share), have placed the organisation in a strong position to implement such programs into the future.
