Prioritising hospital patients at high-risk of medication harm: development and validation of a predictive risk model
Mrs Nazanin Falconer is a pharmacist at the Princess Alexandra Hospital and a PhD Candidate at the School of Pharmacy, The University of Queensland. Nazanin’s research interests include medication safety and risk models to assist with the identification of patients at high-risk of medication harm. Her research explores the development and evaluation of predictive risk tools, using digital hospital data for general medical admissions and older adults. Nazanin hopes that systematically identifying high-risk patients, using near-real time technology to facilitate timely and targeted medication management planning, will reduce harm and optimise patient outcomes in hospitals.
