Information challenges in aged care: Supporting data standardisation and exchange

Older people use health and aged care extensively, often linked to multiple service providers simultaneously. Continuity of care is ideally supported by shared information around medical, functional and psychosocial needs. In this presentation, a series of strategies are proposed to support information sharing among providers, addressing data standardisation, information exchange and decision support.
